Bone Mineral Density and Hip Fracture in Elderly People

Abstract
Purpose : The bone mineral density (BMD) of elderly patients with hip fractures was measured in order to estimate the relationships with other groups, and to suggest a reference value for starting an osteoporosis treatment program in order to decrease the incidence of the hip fracture, if possible.

Materials and Methods : Forty-eight hip fractures associated with osteoporosis were examined. BMD was measured by DEXA (Hologic QDR-4500W, S/N 49492) in the proximal femur and lumbar spine.

Results : BMD of the hip fracture patients was decreased compared to that of normal and agematched people (femur total : 0.555 g/§², T-score = -3.3, Z-score = -1.5). BMD of the neck and Ward¡¯s triangle area in the femur neck fracture group was significantly decreased compared to that in the intertrochanteric fracture group (p<0.05). The severity of osteoporosis did not affect fracture displacement in femur neck fractures, or the stability in intertrochanteric fractures. The fracture threshold in the overall hip fracture group was -1.8 of T-score.

Conclusion : BMD of elderly hip fracture patients was decreased compared to that of normal and age-matched people, and a T-score less than -2.0 could be considered as a minimal criterion for starting an osteoporosis treatment program.
